TABLE SIGN: Please indicate exactly how you would like table sign to read. If sharing with another hotel, or as a group of hotels by flag, county or CVB, please agree on wording. CC&A will make every effort to cluster tables as a "block" if requested early, in writing, by CVBs or hotel chains.

By my signature below, I (we) agree to the following terms and conditions for the 10^th Annual Chuck Cook & Associates' Trade Show & Reception to be held August 14, 2008, from 5:00 to 7:30 pm at University Center Club, FSU stadium in Tallahassee, FL. By acceptance of our check, CC&A agrees to reserve our table for this event, and that this application and the accompanying rules & regulations constitutes a valid and binding contract between us and CC&A.
