TABLETOP FEE: Each six-foot table, skirted, with sign is $650. Eight-foot table, skirted, with sign is $900

A 50% refund will be granted if cancellation is received before June 30th.   NO refunds after June 30th.
TABLE SIGN: Please indicate exactly how you would like table sign to read.  If sharing with another hotel, or as a group of hotels by flag, county or CVB, please agree on wording.  CC&A will make every effort to cluster tables as a "block" if requested early, in writing, by CVBs or hotel chains.

ACCEPTANCE OF AGREEMENT
By my signature below, I (we) agree to the following terms and conditions for the 11th Annual Chuck Cook & Associates' Trade Show & Reception to be held August 13, 2009,  from 5:00 to 7:30 pm at University Center Club, FSU stadium in Tallahassee, FL.  By acceptance of our application, we hereby agree to pay by check the appropriate fees to CC&A within thirty (30) days of submitting our application. All checks must be received before August. CC&A agrees to reserve our table for this event, and that this application with a check constitutes a valid and binding contract between us and CC&A.
